YOu have to get those items at the locations on the map where the paticular quest starts. Wardens Keep , gives you the warden commander armor. Awakening has all new armor and new levels. If you can't find a certain piece of gear then it's usually sold by bodhan. Especially the cool looking blood dragon armor.
Dragon Age: Inquisition wasn't the first game in the franchise to release packs in this way as even Dragon Age 2 released smaller DLC that only contained armor. These took on the name of the All
The fact you can't use dlc items (that you paid for) during the rest of the game is actually bullshit imo. Maybe their way of getting the items in there with the highest quality is to make the skin of the items look just like it. And, if you don't want it any more, you can just delete the dlc (I don't remember if that was how you fix it).
An Awakening file can go into Golems and Witch Hunt but not Origins. A Golems file can only be imported into With Hunt and Witch Hunt can not be imported into anything (well DA2 but that is not out yet). So to do everything on one character you have to play Origins, Awakening, Golems then Witch Hunt in that order. Yes, it is a truly awful system.
For the most part, I think I'll be fine without some of the items, probably, my main worry is that the actual Other Campaigns won't register to Awakening if I don't use the DLC transfer stuff. Then scroll right until you can select Witch Hunt. We're having a great time: food, wine, a little personal abuse, and the universe going foom.
First of all, I don’t own any of the promo items because I only tried getting them after “the servers stopped working for Origins.”. Which is fine by me because the promos weren’t really worth it imo. However, I do have several if not all of the promo items for DA2.
Then you're prompted to go to the Circle Tower at a certain point in Redcliffe so that's where I go next. Then back to Redcliffe. Then the Stone Prisoner DLC. Then Orzammar. Then the Warden's Keep DLC. Denerim sidequests can go here. Then the forest. Then the Return to Ostagar DLC. Then the Urn of Sacred Ashes.

All the other DLC are standalone which you do can do at any time but if you intent to import your save from one to the next you have to do so in a specific order. Origins -> Awakening -> GoA -> Witch Hunt. Each module creates it's own save that you use to import into the next. You can skip a stage but you cannot will not be able to go back.
